At Florida Hospital Memorial Medical Center, medical nutrition therapy is available on both an outpatient and inpatient basis.

The service is provided by registered and state certified dietitians with specialized training in the role of food and nutrition, the prevention of and therapeutic management of disease.

Look for the initial RD after your nutritionist’s name.  These credentials indicate that the practitioner has completed an accredited 4 year educational program, 900 hours of supervised practice and has passed a national exam.

In most states such as Florida, dietitians and nutritionists are licensed or certified.  The initials CD, LD or LD/N indicate that the nutritionist is licensed to practice Medical Nutrition Therapy.  In addition, many nutritionists have specialty credentials.

Outpatient Medical Nutrition Therapy

FHMMC offers medical nutrition therapy that includes comprehensive evaluation and focused education, used to treat nutrition problems that are diagnosed by a physician.

Services Include:
  • Review of personal eating habits and food preferences.
  • Assessment of how these eating habits affect specific health concerns.
  • Written and verbal instructions on improving eating habits to meet personal nutrition goals or a physician's diet prescription.
  • Practical tips and educational materials for assistance with food shopping and preparation, eating in restaurants, and changing dietary habits conveniently.

To schedule an appointment, please call 386-231-3174, Monday-Friday from 8:00 - 4:00 p.m.  All appointments must be scheduled in advance.

For an individual appointment, a physician referral is required by billing insurance carriers -- providing that Medical Nutrition Therapy is a covered benefit in the plan.

What to Bring

A signed referral from your doctor, if it has not already been mailed or faxed to the Dietitian. By law, you must have a written doctor's referral to receive medical nutrition therapy.

A copy of recent lab results (non-FHMMC  patients only).   Alternatively, you may ask your doctor to fax labs results to us at (386-231-3528)

A three-day food record — a written record of everything you eat and drink for three days — if possible.
