Health Information Management Services (formerly the Medical Records Department) maintains records for all inpatient, same-day surgery and outpatient services for both the Florida Hospital Memorial Medical Center and Oceanside campuses. Access to records is protected by federal HIPAA regulations. Copies of records may be released upon receipt of written authorization of the patient (if over 18 years of age) or the parent/guardian. The authorization must:
• Include the name and address of the person receiving the copies
• Specify the information to be released (including date(s) of service)
• Be signed and dated by the patient or parent/guardian
• Be addressed to the proper facility location
In emergency situations, information can be faxed.
